What to Expect at Your First Visit

To help us prepare and make your visit as smooth as possible, please review the following information carefully.—

New Patient Forms & Insurance Information

Completing your new patient forms and providing accurate insurance information as soon as possible is very important.

• Digital Forms: Our secure digital forms will be sent to you by text message or email shortly after your appointment is scheduled. Please complete them right away. These forms include your dental insurance details, which allow us to verify your benefits and check for any previous dental history on file with your insurance plan.

• Paper Forms: If you prefer to fill out your forms in person, please arrive 15 minutes before your appointment and send us your insurance details ahead of time so we can verify your coverage before your visit.
